Abstract

Preterminfantshavehigherriskofacuteandchronichealthcomplica-tionsduetoimmatureorgansincludingrespiratory,immune,andgas-trointestinalsystemswhichareaffectedthemost.Preterminfantshavesensitiveandfragilegutandhencetheenteralfeedsforprematurein-fantsneedstobepreciseandsafe.Cow’smilkproteinallergy(CMPA)isthemostcommonfoodallergiesininfants,CMPAinapreterminfantcouldleadtoseriouscomplicationsduringtheNeonatalIntensiveCareUnit(NICU)stay.Bovinemilkconsistsofsubstantialquantitiesofallergenssuchasß-lactoglobulinandseveralotherbiomoleculeswithallergicpotential,thesameareinvariablyfoundinbovinemilk-basednutritionalproductsincludingbovinemilk-basedfortifiers(BMBF)andformulaswhichcanleadtoCMPAinpreterminfants.HerewepresentacasereportofCMPAinagirlbabybornat30weeksgestationweighing1500gm,admittedinNICUwithbirthasphyxiaandsevererespiratorydistresssyndrome(RDS)BabydevelopedCMPAtoBMBF’sinvolvingseverefeedintoleranceepisodesandpoorweightgain(5g/day).TheCMPAwasmitigatedbyswitchingtohumanmilk-basedfortifier(HMBF),whichresultedinceasingthefeedintoleranceandprovidinganeffectiveweightgain(30g/day).HMBFcanbeconsideredasasafeandeffectivefortificationoptionforpreterm infants
