The overall 5-year survival in nonmetastatic one for totally resected TCs is about one third and for nonoperated one fourth of patients. From here on, it is accepted that, surgery is the first step in treatment. During diagnosis, 30% of these tumors are disseminated. The survival rate of the last group of patients is unknown, as is the best treatment approach.</p> <p><em>Purpose.</em> To better understand the possibility of treatment for DTC.</p> <p><em>Subject.</em> We report a case of primary disseminated TC (DTC) stage T1N2M1 (bra, lym, hep, oth) resistant to cytostatics, but amenable to radiotherapy. Based on the multiple uses of hypofractionated image-guided stereotactic body ablative radiotherapy (IG-SABR), we were able to maintain the patient more than for five years with good quality of life. We demonstrated that radiotherapy is the most efficacious remedy against TC.</p> <p><em>Conclusions.</em> Contemporary methods of irradiation and use new radiobiology data of hypofractionation in metastatic disease provide the possibility to achieve long survival of such patients with good quality of life, even in the T1N2M1 stage. Disseminated thymic carcinoma should be investigated separately from other stages and according to the TNM staging system. Bacterial pathogens are usual suspects; however, viral cause should not be overlooked. We report a case of Parechovirus neonatal meningitis in an 8 days old male infant, emphasizing the importance of considering uncommon viral pathogens in the diagnostic work up. Human Parechovirus belongs to the family of Picornaviridae virus. It is very uncommon and has been associated with a benign clinical course of respiratory and gastrointestinal infection in most cases, however, it is emerging as one of the causes for emergency admission of neonates in hospital due to certain serotypes. The HPeV type 3 has been found mostly in children and young infants presenting with meningitis and sepsis like illness. Infants with severe Central Nervous System (CNS) infections are at an increased risk of long-term sequelae. In this case we describe the clinical presentation, diagnostic challenges, management and outcome of this rare infection. An 8 days old baby presented to our emergency department with sepsis like illness. He was admitted in neonatal unit for further investigations and management. Despite initial presentation the inflammatory markers were normal. Symptomatic-supportive treatment was given till baby made full recovery. Antibiotics were discontinued after 48 hours as culture was negative for bacteria. A 23-year-old female was referred to our clinic with symptoms including headache, nausea, altered mental state, and loss of coordination. She had a significant medical history for Psychiatric illness, Endometriosis, and Polycystic Ovarian Syndrome. She had been turned down by numerous specialists before being diagnosed with ABS once consulting with an Infectious Disease Physician, through breathalyzer test results and IV Micafungin treatment.
